A 45-year-old woman developed Mazzotti reaction following treatment with ivermectin for scabies. The woman was administered oral ivermectin 200 µg/kg for scabies; around 20 hours later, a rash developed on the forearms, which later spread to her neck, abdomen, thighs and chest. After 6 days, exacerbation of maculopapular, itchy, eruptions was noted predominantly affecting the interdigital regions, despite administration of levocetirizine and crotamiton. Corticosteroids were later initiated, and after 1 week, urticarial macules persisted on the legs with an eosinophil value of 11.5% up from 0.1%. Five weeks after initial presentation, complete symptom resolution was noted.
