A 36-year-old man developed heparin-induced thrombocytopenia (HIT) and HIT-associated disseminated intravascular coagulation during treatment with heparin [DIC; not all dosages clearly stated]. He subsequently developed progressive thrombosis due to under-anticoagulation from PTT and ACT confounding and died.
The man underwent aortic valve replacement and aortic root and sinus of valsalva enlargement and received heparin 40 000U [route of administration not stated]. On postoperative day 1, he received SC heparin 5000U and an additional two doses of SC heparin 5000U on postoperative day 2. On postoperative day 4, he started receiving a heparin infusion (1500 -2500 U/h) for left femoral vein deep-vein thrombosis (DVT) with interruptions for procedures, and additional heparin 3000U boluses on days 4, 6, 7 and 8.
