A 65-year-old woman developed encephalopathy while receiving phenytoin and cotrimoxazole [sulfamethoxazole/trimethoprim; routes not stated].
The woman had non-small cell lung cancer with brain metastases and started receiving phenytoin 300mg daily for seizure prophylaxis. About 5 months later, she was receiving phenytoin 450mg daily. About 5 months later, she developed aphasia, incontinence, spasticity, increased motor tone with clonus in deep tendon reflexes, delayed swallowing, disorientation and apraxia.
