RATIONALE The short supply of donor organs has been one of the most critical problems in the area of lung transplantation, and this is especially serious in Japan. One approach to attempt to address this limitation is the use of extended criteria donor lungs. In Japan, lungs were used for transplantation in more than 60% of deceased donors. Therefore, it can be easily imagined that variance from standard donor guidelines may be fairly frequent. However, degree of compliance with donor guidelines and the effect of guideline variances on graftsurvival have not been analyzed in Japan.
