A 14-year-old female to male transgender patient developed methaemoglobinaemia secondary to drug toxicity, vulvular irritation and labia enlargement following benzocaine/resorcinol misuse and overdose, while using it for the treatment of vulval itching and burning. In addition, the patient exhibited lack of efficacy during treatment with clobetasol for chronic eczematous dermatitis [dosages and times to reaction onsets stated]
The patient presented with vulvar discomfort since several months. The patient described initial symptoms of irritation and pruritus, followed by significant enlargement of both labia, with the right labium larger than the left. The patient was not able to wear underwear or pants due to discomfortness. The patient mentioned thick yellow discharge from the vagina and constant weeping from the labia majora. The patient was previously assessed in the community, where biopsy revealed chronic eczematous dermatitis consistent with changes seen in a chronic inflammatory process. The patient tried topical 0.05% clobetasol ointment for near about 3 months without any benefit (lack of efficacy)....
