Abstract
A Paralisia Cerebral descreve um grupo de distúrbios permanentes do desenvolvimento do movimento e da postura, com limitação funcional, atribuídos a insultos não progressivos a um cérebro em desenvolvimento. Considerada uma das principais causas de incapacidade física em idade pediátrica, a Paralisia Cerebral associa-se a múltiplos fatores de risco. As diversas manifestações clínicas dependem da localização da lesão cerebral e da idade da criança e permitem a classificação da Paralisia Cerebral em Espástica, Discinésica e Atáxica. A distribuição anatómica e a severidade das manifestações também representam critérios utilizados na classificação.
Reconhecendo as comorbilidades potencialmente associadas, a atual possibilidade de fazer um diagnóstico precoce permite uma intervenção personalizada atempada e uma monitorização rigorosa e dirigida da saúde da criança.
A inexistência de uma cura torna a melhoria da qualidade de vida, a funcionalidade e a evicção do sofrimento os focos das intervenções terapêuticas. Para isto, existem diversas terapias disponíveis, farmacológicas e não farmacológicas. A fisioterapia, terapia ocupacional, fármacos para controlo sintomático e cirurgias constituem alguns exemplos de tratamentos disponíveis. Sendo o bem-estar a preocupação dos cuidados paliativos, estes desempenham um importante papel na gestão da saúde da criança com Paralisia Cerebral.
Neste trabalho é descrito um caso clínico de um menino de 12 anos de idade diagnosticado, no seu primeiro ano de vida, com Paralisia Cerebral no contexto de encefalopatia hipóxico-isquémica. Desde o seu nascimento, o menino é acompanhado por profissionais de saúde do Hospital Pediátrico de Coimbra, Centro Hospitalar Universitário Cova da Beira e Associação de Paralisia Cerebral de Coimbra. Este acompanhamento ao longo da sua vida permitiu o diagnóstico de diversas comorbilidades, como Síndrome de West, luxação bilateral da anca, escoliose toraco-lombar, refluxo gastroesofágico, disfagia, Síndrome de Apneia Obstrutiva do Sono, desnutrição, infeções respiratórias e otites de repetição.
Como intervenções terapêuticas, o menino frequenta sessões de fisioterapia e terapia ocupacional desde o seu diagnóstico e recebe injeções de toxina botulínica semestralmente, desde 2011. Foi intervencionado em 2012, realizando-se fundoplicatura de Nissen e gastrostomia. Atualmente, é acompanhado por uma equipa intra-Hospitalar de Suporte em Cuidados Paliativos Pediátricos através de visitas domiciliárias, mantém-se estável do ponto de vista neurológico e respiratório, alimenta-se exclusivamente por gastrotomia, sob ventilação não invasiva durante o sono e encontra-se polimedicado para controlo sintomático.
A partir deste caso clínico foi feita uma revisão da literatura sobre a Paralisia Cerebral, referindo-se, especificamente, as diversas medidas a considerar no acompanhamento de uma criança após o seu diagnóstico.
