In 1999 and 2001, the Institute of Medicine issued two landmark reports: "To Err Is Human: Building a Safer Health System" and "Crossing the Quality Chasm: A New Health System for the 21st Century," respectively. Although the first report concerning medical errors received enormous attention, the second less publicized report had more significance for the overall health care of Americans.
